    Vascular health in middle age could add 12.6 years without dementia

    Vascular health as a brain factor

    The health of the cardiovascular system influences brain aging, which requires a constant supply of blood and oxygen. When blood flow decreases or blood vessels lose integrity, brain tissue may be exposed to cumulative deterioration that affects memory, reasoning, and other cognitive functions.

    Over the years, blood vessels can suffer structural damage and promote inflammatory processes. Hypertension adds another source of stress, as high pressure persistently subjects vascular walls to strain and can cause damage to brain tissue.

    Although high blood pressure can be prevented or controlled, its prolonged presence has been linked to a chain of damage that can accumulate long before dementia symptoms appear. Therefore, assessing cardiovascular factors during middle age is relevant for studying subsequent cognitive risk.

    What the follow-up study in the U.S. found

    The study used data from the Atherosclerosis Risk in Communities project, a prospective cohort with participants from four communities in the United States. Researchers analyzed individuals who were alive and did not have dementia at age 55, using measurements taken during the second visit of the project, between 1990 and 1992.

    The sample included 12,409 participants with a mean age of 56.2 years and a standard deviation of 5.2 years. The follow-up had a median of 26.3 years, during which 3,008 people developed dementia and another 5,238 died without having received that diagnosis.

    To calculate the burden of vascular risk, the analysis counted three conditions: diabetes, hypertension, and current smoking. Diabetes was identified through self-report, medication use, or an HbA1c of 6.5% or higher, while hypertension corresponded to a blood pressure of at least 140/90 mm Hg or the use of antihypertensive medication.

    The methodology did not present these elements as a unique explanation for each individual case, but rather as a way to measure the accumulation of risks in middle age. The study observed associations within a population followed for decades, not a guarantee that a specific person will avoid or develop dementia based on their personal count.

    The difference between zero and three factors

    Compared to participants without risk factors, those with all three showed a significantly higher risk of dementia. The hazard ratio (HR) reached 2.69 for dementia, indicating a substantial association between the accumulated vascular burden and the cognitive outcome observed during the follow-up.

    The relationship also appeared in mortality without dementia, with a hazard ratio (HR) of 5.61 among those with hypertension, diabetes, and smoking. This result shows that vascular burden was linked both to the possibility of cognitive decline and to a higher likelihood of dying before dementia could be diagnosed.

    The analysis incorporated this mortality as a competing event, a necessary consideration when studying years of life without dementia. If a person dies before developing the disorder, they are no longer exposed to that diagnosis; therefore, death and dementia must be interpreted together when estimating health trajectories.

    The most expressive difference appeared in life expectancy without dementia. Participants without risk factors reached an estimate of 30.1 years of life without dementia, while those with all three factors recorded 17.5 years, a gap of 12.6 years associated with the worst vascular category.

    Causes of Recent Movements

    In this case, it is not appropriate to attribute the observed movement to a market catalyst: the content pertains to a medical study and does not present recent price variations or a market cause that needs to be explained.

    Implications for Prevention

    The results reinforce the idea that brain prevention begins much earlier than when a person notices memory failures or changes in their autonomy. Middle age appears as a stage where blood pressure, metabolic control, and tobacco consumption can influence the vascular reserve that will support the brain during aging.

    Blood pressure stands out because the study defined it both by a measurement of 140/90 mm Hg and by the use of antihypertensive medications. This definition acknowledges that a person under treatment can still be part of the population with hypertension and avoids interpreting medical control as a historical absence of the risk factor.

    Diabetes was also evaluated with several criteria, including HbA1c, a measure related to average blood glucose. By adding self-reporting, medication, and the threshold of 6.5%, the researchers sought to capture the condition more broadly within the available records.

    Current smoking completed the studied combination, but the analysis did not present the three factors as equivalent nor claimed that each produces exactly the same damage. Its main message was cumulative: as the number of vascular risks present in middle age increased, the expected years without dementia decreased, and adverse outcomes increased.

    A Long-Term Message

    The research, published under the title "Greater Vascular Health in Mid-Life Reduces Later Risk of Dementia," adds to the body of work linking vascular health with cognitive decline. Its results indicate that better vascular health during middle age was associated with prolonged survival without dementia.

    The long-term follow-up offers a different perspective than studies that only observe participants for a few years. A median of 26.3 years allows connecting conditions recorded around age 55 with events occurring later, although the duration does not eliminate the inherent limitations of an observational study nor does it convert the association into proven causality.

    The finding of an additional 12.6 years of life without dementia should be read as an estimated difference between risk categories, not as an individual promise of cognitive longevity. The figure combines the higher risk of dementia with a notably higher competitive mortality, so it does not simply represent the delay of a diagnosis in individuals who will necessarily live longer.

    Overall, the study points to a practical and sober conclusion: maintaining optimal vascular health during middle age is associated with a more favorable trajectory for the brain. The challenge for prevention is to identify and control these factors well in advance, before accumulated vascular injuries reduce the chances of reaching advanced ages without dementia.
